Terms Of Participation
You are invited to participate in a pilot program on Destigmatizing mental health among the Black and African diaspora community in Utah (ages 18 and above) in Utah. We ask that you read this form before agreeing to be in the project.
This Pilot project is being conducted by GK Folks Foundation’s Mental Health program.
Background Information and Procedures:
The purpose of this project is to increase mental health awareness and reduce the stigma associated with mental illness in immigrant and refugee communities. To do this, we are implementing a pilot project that will consist of a series of 5 online discussion-based workshops. Each session will comprise two components: (1) Storytelling, featuring a black immigrants’ personal experiences dealing with mental health issues; (2) A 50-minute discussion on mental health facilitated by a licensed mental health professional. This 50-minute discussion will include a brief introduction to mental health, address perceived barriers to accessing mental health services, discuss the benefits of help-seeking behavior, provide self-care recommendations and promote self-efficacy by sharing local mental health resources. If you agree to be in this project, we would ask you to complete an online survey about complete a survey to assess current beliefs and attitudes toward mental health and mental health service utilization. After the 5-session discussion-based workshops, participants will be asked to complete a second survey to assess their behavioral intentions based on new knowledge gained. No identifying information will be retained and your participation in the project will be complete.
Risks and Benefits of Participating in this project:
There are no known risks to you participating in this project beyond those that exist in daily life. A benefit to you is, you may experience an indirect or direct benefit in the form of increased insight and awareness into Mental Health
As compensation for your time and effort in completing all 5 workshops, you will receive a $25 gift card, if you wish, you will be entered into a random prize drawing to win the grand prize of $200. In order to participate in the random prize drawing, you will be asked to submit an email address at the conclusion of the survey so that you can be contacted in the event that you are selected. Your email address will be stored separately from your survey responses. The winners will be randomly selected and will be contacted via email by December 15th, 2020, and receive their prize by Dec 22nd, 2020.
The records of this project will be kept private. In any sort of report, we might publish, we will not include any information that will make it possible to identify a subject. Program records will be stored securely and only researchers will have access to the records. Program data will be encrypted according to the policy for the protection of confidentiality.
Voluntary Nature of the Project:
Participation in this project is voluntary. Your decision whether or not to participate will not affect your current or future relations with the Foundation. If you decide to participate, you are free to withdraw at any time without affecting those relationships. If you do not wish to complete this project, just close your device’s window.
Statement of Consent:
I have read the above information and am giving my consent to participate in this pilot project. I certify that I am a Black Immigrant, ages 18 and above, residing in Utah, and, by clicking the ‘yes’ button below to enter the Project, I indicate my willingness to voluntarily take part in the Pilot program
